Here you could measure the following KPIs:

Total money loss due to teller error over given time period (e.g. one quarter, one year)
Sum of total money loss

Total money loss due to teller error per bank teller over given time period
Total money loss due to teller error / Total number of tellers

Depending on your other KPIs and what is affected by this, you could create a wide range of other KPIs based on the ones above. For instance, you could also calculate:
Total money loss due to teller error as a % of total bank net profits over given time period
Total money loss due to teller error / total bank net profit x 100

If your scope is to identify the reason/type of the error and create strategies to minimize it you could try identify the factors for teller error and measure KPIs based on those.

Let’s assume factors are:
- Teller negligence (though this can be a bit broad, there may be further reasons behind it)
- Tiredness (too few tellers per branch? Overtime hours? too little lunch breaks? Etc.)
- Customer behaviour (rude customer making the teller nervous?)
- System failure (faulty computer system, faulty machine tape? )
- Etc
One method would be to measure the % of the total money loss due to teller error per every identified factor. Based on this you could create strategies to minimize teller error and hence money loss.
