I guess the first approach is to divide heath and safety problems into different levels, for instance number of some minor and major accidents.

The next idea is building KPIs basing your security and safety policies. I'm sure that you already have safety policies, select from them some key requirements and add KPIs that will be able to measure and control these important requirements.
