As for specific engagement signals related to the continuous improvement culture, I think you will need to do your own research (taking company's data as a source) and come up with some leading indicators.

Few examples of good/bad indicator:
Bad indicator: the number of defects/problems. Why it is bad? Problems happen. You cannot avoid it and the level of problem is a poor indicator of continuous improvement culture.

Good indicator: the number of returning defects/problems. If your team is really learning from the mistakes, then you'll see that the number of the returning problems is decreasing.
