The best approach to the improving patient outcomes is to start with root-cause analysis to find out what causes some typical problems. Once those reasons are identified it is a good idea to build a strategy map with goals that will address those problems and align them with respective lagging and leading indicators.
Unfortunately it is hard to outsource the analysis part, but your root-cause analysis and respective KPIs can be formalized using BSC Designer software.
